Each month one (1) Winner will be selected AT RANDOM from all the COMPLETED SURVEYS across all our Midas
locations for the previous month. The winner selected will receive a Midas Gift Card to be used at only one of the
Midas locations owned by Auto Systems Centers, Inc. The Midas Gift Card will expire Two (2) Years from Date of Purchase. The Midas Gift Card
is not redeemable for cash. The provider of this card is not responsible for lost, stolen, or damaged cards. Not

To Enter this monthly drawing, you must enter a valid Midas location and enter that 3-digit Shop Number as well as
the full 7-digit invoice number on your invoice. You may take the survey and will only be entered into the drawing once
per invoice (multiple entries using the same shop and invoice number WILL NOT increase your chances of winning). Entries
must be completed and submitted by 11:59 PM EST on the last day of the month to be entered in the next month's drawing. We
reserve the right to disqualify any surveys and contest entries for any reason whatsoever.
